Patient Bill of Rights

Information Disclosure
You have the right to receive accurate and easily understood information about your health plan, health care professionals, and health care facilities. If you speak another language, have a physical or mental disability, or just don't understand something, assistance will be provided so you can make informed healthcare decisions. You have the right to be informed of financial benefits when referred to another organization for service and to be informed of patient rights under state law to formulate an Advance Directive, if applicable. You have the right to be able to identify visiting staff members through proper identification. You have the right to identify the staff member of the program and their job title, and to speak with a supervisor of the staff member, if requested. You have the right to speak to a healthcare professional. You have the
right to be fully informed of one’s responsibilities.

Choice of Providers and Plans
You have the right to a choice of health care providers (pharmacies) that is sufficient to provide you with access to appropriate high-quality health care.

Access to Emergency Services
If you have severe pain, an injury, or sudden illness that convinces you that your health is in serious jeopardy, you have the right to receive screening and stabilization emergency services whenever and wherever needed, without prior authorization or financial penalty.

Participation in Treatment Decisions
You have the right to know all your treatment options and to participate in decisions about your care. Parents, guardians, family members, or other individuals that you designate can represent you if you cannot make your own decisions. You have the right to be informed, both orally and in writing, in advance of care being provided, of the charges, including payment for
care/service expected from third parties, and any charges for which the patient will be responsible. You have the right to receive information about the scope of services that the organization will provide and specific limitations on those services.

Respect and Nondiscrimination
You have the right to considerate, respectful and nondiscriminatory care from your doctors, health plan representatives, and other health care providers. You have the right to have your property and person treated with respect, consideration and recognition of patient dignity and individuality. You have the right to be free from mistreatment, neglect, or verbal, mental, sexual and physical abuse, including injuries of unknown source, and misappropriation of patient property.

Confidentiality of Health Information
You have the right to talk in confidence with health care providers and to have your health care information protected. You have the right of confidentiality and privacy of all information contained in the patient record and of Protected Health Information. You also have the right to review and copy your own medical record and request that your physician amend your record if it is not accurate, relevant, or complete. You also have the right to be advised on Proprium
Specialty Apothecary policies and procedures regarding disclosure of clinical records, and to have personal health information shared with other healthcare providers only in accordance with state and federal laws

Complaints and Appeals
You have the right to voice complaints regarding treatment of care, lack of respect of property or recommend changes in policy, personnel, or care/service without restraint, interferences, coercion, discrimination, or retaliation. You have the right to a fair, fast and objective review of
any complaint you have against your health plan, doctors, hospitals or other health care
personnel. This includes complaints about waiting times, operating hours, the conduct of health care personnel, and the adequacy of health care facilities.
